Pre-Op Safety Check

An office-based oral-surgery pre-op that turns a patient's history and vitals into ranked, surfaced risk flags — before anyone is sedated.

Patient age

Age drives sedation dosing and clearance. Age ≥ 70 adds a caution flag; ≥ 80 also raises the estimated ASA class.

Airway — Mallampati class

The Mallampati score (I–IV) grades how much of the throat is visible and predicts a difficult airway. Class III is a caution; class IV is a high-severity flag.

Cardiac history

Stable disease (e.g. controlled hypertension / CAD) warrants closer monitoring; a recent cardiac event is high-severity and usually needs cardiology clearance first.

Fasting status (NPO)

How long since the patient last ate or drank. Sedation on a full stomach raises the risk of aspiration. Guidelines are roughly ≥ 6 h after a light meal and ≥ 2 h after clear liquids. "Ate recently" raises a high-severity flag.

History & medications

Obstructive sleep apnea (OSA)

OSA raises the risk of airway obstruction and desaturation under sedation. Titrate conservatively and keep airway support ready.

On anticoagulant / antiplatelet

Blood thinners increase bleeding risk. Coordinate a hold/bridge plan and have local hemostatic measures ready.

Known bleeding disorder

A disorder such as von Willebrand or hemophilia may need factor support and hematology coordination.

Prior adverse sedation/anesthesia event

A prior bad reaction predicts higher recurrence risk — identify the agent and consider an anesthesiology consult.

Pregnant

Elective office sedation in pregnancy carries added maternal-fetal risk and is usually best deferred.
